Job summary

Alternate Solutions Health Network is seeking a Licensed Practical Nurse to provide skilled nursing care in patients' homes, following physician orders and RN-developed care plans. You will educate patients and families, administer medications, and document care in accordance with Medicare guidelines.

Three days per week, 8:15 AM–5:00 PM. Requires an active LPN license in Michigan and 1+ year of acute care experience; home health experience preferred.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum of one year of experience as an LPN in an acute care setting; two years preferred.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians, and coworkers.

Responsibilities

• Provide skilled nursing care in a patient’s home as ordered by the attending physician.
• Administer medications as prescribed and monitor effects.
• Educate patients and families on disease processes and care plans.
• Complete clinical documentation per agency and Medicare guidelines.
• Maintain active LPN license and comply with regulations.
• Coordinate with scheduler and participate in on-call duties.

The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.KEY RESPONSIBILITIESProvides skilled nursing care in a patient’s home as ordered by the attending physicianPerforms care by following order sets and goals made by RNObserves and monitors patient conditionsNotifies RNCM of any changes in patient conditionAdministers medication as prescribed by the physicianCounsels patient and family on the disease/injury processes and how to manageIncorporates patient and family in development of plan of careHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationMaintains active LPN license in state of employmentCommunicates with scheduler any changes outside the normalParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresDrives to patient’s primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.Other duties as assigned.QUALIFICATIONSMinimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
