Job summary

Alternate Solutions Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients’ homes across the assigned territory in Virginia. You will follow physician orders, administer medications, educate patients and families, and document care per Medicare guidelines.

Applicants should have an active LPN license in the state, a valid driver’s license, and be able to work within a team. A $10,000 sign-on bonus is offered for eligible candidates.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum of one year of experience as an LPN; two years preferred.
• Home care experience is desired.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions; notifies RN of changes.

The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.KEY RESPONSIBILITIESProvides skilled nursing care in a patient’s home as ordered by the attending physician.Performs care by following order sets and goals made by RN.Observes and monitors patient conditions.Notifies RNCM of any changes in patient condition.Administers medication as prescribed by the physician.Counsels patient and family on the disease/injury processes and how to manage.Incorporates patient and family in development of plan of care.Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.Listens to patient and family members.Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.Documents all aspects of treatment, assessments, and patient education.Maintains active LPN license in state of employment.Communicates with scheduler any changes outside the normal.Participates in all on-call requirements and case conferences.Attends in-service trainings and mandatory agency meetings.Maintains a professional appearance as a representative of the company.Understands and practices agency policies and procedures.Drives to patient’s primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.Other duties as assigned.QUALIFICATIONSMinimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Ability to remain calm, have patience and be accommodating.
