Alternate Solutions Health Network – Adena Home Care is seeking an LPN I to provide skilled nursing care in patients' homes, following physician orders and RN-created care plans. You will educate patients and families and ensure safety throughout the visit.

This role requires a valid LPN license in the state, 1 year of acute care experience (2 years preferred), strong communication, driving flexibility, and adherence to Medicare guidelines and agency policies.

Qualifications

  • Must have active LPN license in the state of employment.
  • At least 1 year of LPN experience in an acute care setting; 2 years preferred.
  • Home care experience is desired; driving to patient locations required.

Responsibilities

  • Provide skilled nursing care in patients' homes as ordered by the attending physician.
  • Follow order sets and goals created by the RN.
  • Administer medications as prescribed by the physician.
  • Document all aspects of treatment, assessments and patient education per Medicare guidelines.
  • Notify RNCM of changes in patient condition and participate in on-call requirements.

The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.KEY RESPONSIBILITIESProvides skilled nursing care in a patient’s home as ordered by the attending physicianPerforms care by following order sets and goals made by RNObserves and monitors patient conditionsNotifies RNCM of any changes in patient conditionAdministers medication as prescribed by the physicianCounsels patient and family on the disease/injury processes and how to manageIncorporates patient and family in development of plan of careHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationMaintains active LPN license in state of employmentCommunicates with scheduler any changes outside the normalParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresDrives to patient’s primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.Other duties as assigned.QUALIFICATIONSMinimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Ability to remain calm, have patience and be accommodating.
