Alternate Solutions Home Health of Columbus is seeking a Licensed Practical Nurse (LPN I) to provide comprehensive skilled care in patients' homes, following physician orders and RN-directed care plans. The role emphasizes patient and family education, accurate documentation, and adherence to Medicare guidelines.

The ideal candidate has an LPN license, at least 1 year of acute care LPN experience (2 years preferred), and strong communication and organizational skills.

We CARE for our patients like they are our own FAMILY.**Agency**: Alternate Solutions Home Health of Columbus**Schedule**: Monday - Friday, 8:15AM - 5:00PM**SUMMARY**The Licensed Practical Nurse (LPN) I provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.**KEY RESPONSIBILITIES*** Provides skilled nursing care in a patient's home as ordered by the attending physician* Performs care by following order sets and goals made by RN* Observes and monitors patient conditions* Notifies RNCM of any changes in patient condition* Administers medication as prescribed by the physician* Counsels patient and family on the disease/injury processes and how to manage* Incorporates patient and family in development of plan of care* Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions* Listens to patient and family members* Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines* Documents all aspects of treatment, assessments, and patient education* Maintains active LPN license in state of employment* Communicates with scheduler any changes outside the normal* Participates in all on-call requirements and case conferences* Attends in-service trainings and mandatory agency meetings* Maintains a professional appearance as a representative of the company* Understands and practices agency policies and procedures* Drives to patient's primary location per scheduled visit.* Daily attendance at assigned visit locations.* Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.* Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.* Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.* Other duties as assigned.**QUALIFICATIONS*** Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
