Job summary

Freeman Regional Health Services, located in Freeman, SD, is seeking an LPN for long-term care. This full-time position involves day and night shifts, patient assessment, medication administration, and coordinated care with the healthcare team.

Qualifications

  • Licensed Practical Nurse (LPN) with SD Board of Nursing certification.
  • 1–3 years of clinical or long-term care experience preferred.
  • Ability to work day and night shifts, including rotating weekends/holidays.

Responsibilities

  • Regularly evaluating residents' physical and mental health, including vital signs, cognitive function, and overall well-being.
  • Accurately dispensing and monitoring medications according to physician orders.
  • Maintaining open communication with residents, families, physicians, and other healthcare team members to ensure coordinated care.
  • Observing for any significant changes in resident health status and promptly reporting them to the appropriate healthcare provider.
  • Assessing pain levels and administering pain medication as prescribed.
  • Providing emotional support and addressing psychosocial needs of residents.
  • Maintaining accurate and detailed resident records, including progress notes and medication administration logs.
  • Collaborating effectively with CNAs and other healthcare professionals to deliver comprehensive care.

Day Shift (6:00 a.m. to 6:30 p.m.)

Night Shifts (6:00 p.m. to 6:30 a.m.)

Rotating weekends/holidays

Becoming an LPN, in Long Term Care (LTC), allows you to provide significant, ongoing care to elderly patients. You can develop deep connections with residents and their families, getting to know their stories and personalities over time, while making a positive impact on their quality of life. By providing attentive care, you can significantly improve the daily lives of residents, making a noticeable difference.

What You Will Do:

  • Collaborating effectively with Certified Nursing Assistants (CNAs) and other healthcare professionals to deliver comprehensive care.
  • Comply with all aspects of resident rights including confidentiality and privacy.
  • Lead, by example, following Freeman Regional Health Services CARE standards.
Required Education, License/Certification, Or Work Experience:
  • Licensed Practical Nurse (LPN) - SD Board of Nursing
  • 1-3 years experience working in a clinical or long term care environment, preferred.
