Job summary

Freeman Regional Health Services in Freeman, SD is seeking a Registered Nurse for long-term care. The role emphasizes direct resident care, medication management, and close collaboration with a patient-centered care team.

You will evaluate residents, monitor vital signs, and document changes while supporting residents and families to enhance quality of life. This full-time position includes day shifts with rotating weekends and holidays.

Qualifications

• Registered Nurse (RN) licensed by the SD Board of Nursing.
• 1–3 years experience in clinical or long-term care preferred.
• Ability to communicate effectively, observe, and document patient status.

Responsibilities

• Regularly evaluating residents' physical and mental health, including vital signs and overall well-being.
• Accurately dispensing and monitoring medications according to physician orders.
• Maintaining open communication with residents, families, physicians, and the healthcare team for coordinated care.
• Observing for significant changes in health status and reporting them promptly.
• Assessing pain levels and administering prescribed pain medication.
• Providing emotional support and addressing psychosocial needs of residents.
• Maintaining accurate resident records including progress notes and medication logs.
• Recognizing changes in resident health due to chronic conditions.
• Collaborating with CNAs and other healthcare professionals to deliver comprehensive care.
• Complying with resident rights including confidentiality and privacy.
• Leading by example to follow Freeman Regional Health Services CARE standards.

What You Will Do

• Regularly evaluating residents' physical and mental health, including vital signs, cognitive function, and overall well-being.
• Accurately dispensing and monitoring medications according to physician orders.
• Maintaining open communication with residents, families, physicians, and other healthcare team members to ensure coordinated care.
• Observing for any significant changes in resident health status and promptly reporting them to the appropriate healthcare provider.
• Assessing pain levels and administering pain medication as prescribed.
• Providing emotional support and addressing psychosocial needs of residents.
• Maintaining accurate and detailed resident records, including progress notes and medication administration logs.
• Recognizing subtle changes in resident behavior or health status due to the chronic nature of conditions.
• Collaborating effectively with Certified Nursing Assistants (CNAs) and other healthcare professionals to deliver comprehensive care.
• Comply with all aspects of resident rights including confidentiality and privacy.
• Lead, by example, following Freeman Regional Health Services CARE standards.

Required Education, License/Certification, Or Work Experience

• Registered Nurse (RN) - SD Board of Nursing
• 1-3 years experience working in a clinical or long term care environment, preferred.
