Job summary

Freeman Regional Health Services in Freeman, SD is seeking a Licensed Practical Nurse (LPN) to join our patient‑centered care team. You will collect data, perform focused nursing assessments, and contribute to the development of care plans under the supervision of a RN or physician.

You will implement interventions, monitor patient responses, advocate for patients, and collaborate with healthcare professionals.

Qualifications

  • Active LPN license in the state of practice is required upon hire.
  • 1–3 years of hospital experience preferred.
  • Ability to work rotating day/evening/night shifts and communicate effectively.

Responsibilities

  • Collects data and conducts focused nursing assessments of patient health status.
  • Participates with the health care team in developing and modifying patient-centered care plans.
  • Implements nursing interventions within the plan of care and evaluates responses to interventions.
