Job summary

Freeman Regional Health Services is seeking a Licensed Practical Nurse (LPN) to join our team in Freeman, SD. This full-time role includes rotating days/evenings/nights, weekends, and holidays, with a minimum of 72 hours per pay period.

The LPN will perform focused nursing assessments, coordinate with the care team, implement the care plan, and advocate for patient safety and education. LPN licensure in the state is required, with 1–3 years hospital experience preferred.

Qualifications

  • LPN license in the state of practice is required upon hire.
  • 1-3 years experience in a hospital setting preferred.

Responsibilities

  • Collects data and conducts a focused nursing assessment of the health status of a patient.
  • Participates with other health care providers in the development and modification of the patient-centered health care plan.
  • Implements nursing interventions within a patient-centered health care plan.
  • Assists in the evaluation of responses to interventions.
  • Provides for the maintenance of safe and effective nursing care rendered directly or indirectly.
  • Advocates for the best interest of the patient.
  • Communicates and collaborates with patients and members of the health care team.
  • Assists with health counseling and teaching.
  • Delegates and assigns nursing interventions to implement the plan of care.

A Brief Overview

The LPN practices under the supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider. The LPN organizes and directs those aspects of care for which he/she is accountable; communicating and coordinating care with well-defined nursing diagnoses, including all health team members.

Required Education, License/Certification, Or Work Experience
  • Licensed Practical Nurse (LPN) - Board of Nursing. An active license, in the state of practice, is required upon hire.
  • 1-3 years experience working in a hospital setting, preferred.
