Job Opportunity: Licensed Practical Nurse (LPN) at BAYADA Home Health Care

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team. This office serves adult clients on a per visit basis in homes throughout Hudson County.

Immediate FULL TIME opportunity available!

Requirements include at least one year of prior clinical experience as a licensed Practical Nurse. As a home care nurse, you will be an essential part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to receive necessary medical care at home.

Responsibilities of a Licensed Practical Nurse (LPN):

• Making home visits to clients in designated areas
• Administering medication, wound care, treatments, and procedures
• Monitoring client conditions and reporting changes to the Clinical Manager and Client Services Manager
• Following and documenting doctors' orders accurately
• Conducting client assessments as needed
• Case management and coordination
• Documenting observations, interventions, and evaluations using electronic medical records on a touchpad tablet

Qualifications:

• Current license as a Licensed Practical Nurse in New Jersey
• Minimum of one year of recent, verifiable LPN experience
• Graduation from an accredited nursing program (transcript or diploma required)
• Prior home care experience is preferred but not required
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
