Summary

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care within the patient's home environment.



Responsibilities

Clinical Competence

  1. Provide high-quality clinical services within the scope of practice defined by state laws, in accordance with the care plan, and in coordination with the healthcare team, following all infection control procedures. Do not alter the plan of care.
  2. Continuously observe and report the patient’s nursing needs to the supervising RN.

Documentation and Care Delivery

  1. Communicate effectively and timely with physicians, patients, family members, and agency staff regarding patient care issues, visit schedules, changes, orders, and documentation such as OASIS data sets and coding requests, ensuring coordinated care and adherence to laws and regulations.
  2. Thoroughly document patient visits according to policy and payer requirements, maintaining accurate medication lists and timely updates.

Quality

  1. Participate actively in performance improvement initiatives, aiming to reduce hospitalizations and improve patient outcomes.

Professionalism (timeliness, training, compliance)

  1. Attend and participate in all meetings, including case conferences, punctually, with camera on during video meetings.
  2. Arrive at work with a positive attitude, assist patients and the agency as needed, and be open to learning new skills.

Teamwork (availability, productivity)

  1. Maintain schedule flexibility to provide care as needed, including participation in weekend rotations and observed holidays.
  2. Adhere to and participate in the agency's Episode Management process.
  3. Perform all other duties as assigned.


Education and Experience

License Requirements

  • Current LPN licensure in the state of practice.
  • Valid driver’s license, vehicle insurance, and access to reliable transportation.
  • CPR certification required.
