HIRING NOW. We are seeking an experienced home health RN or LPN with excellent clinical skills to join our Advanced Home Health team. Our nurses can earn up to $80K per year!

LPNs earn between $29 and $35 per visit.

Position: Licensed Practical Nurse (LPN) (Home Health)

Status: Full-Time or Part-Time

Location: Baldwin, PA, South Hills of Pittsburgh

Position Description:

• The LPN will coordinate home care for chronically ill patients using a disease management process, the OASIS assessment, plan of care, and established protocols.
• Will educate patients and caregivers on illness management and promote home health support systems to reduce re-hospitalizations and emergency hospital visits.

Qualifications / Requirements:

• Education: LPN with current PA state license.
• Experience: Home health experience preferred.
• Ability to work independently and in a multidisciplinary team. Valid driver’s license required.
