Licensed Practical Nurse (LPN) Field Nurse - Home Health Visits

ChristianaCare is seeking a dedicated Licensed Practical Nurse (LPN) for field nurse home health visits in Dover, DE. This full-time position supports the Dover area and surrounding communities, including Frederica and Smyrna. The role involves working day shifts, one weekend per month, and two holidays annually. Eligible candidates may receive a $5,000 sign-on bonus.

Responsibilities include:

1. Providing skilled nursing care and prescribed treatments to geriatric patients in the community.
2. Administering care such as Cardiac Care, Foley Catheters, Pulmonary Care, Ostomy Care, among others.
3. Educating patients and families on care procedures, disease management, and nutrition.
4. Participating in discharge planning for patients.

Minimum requirements:

• Current DE LPN license or a compact state license.
• At least one year of LPN experience.
• Previous experience in home healthcare and medical-surgical nursing is preferred.
