Affirms Home Care in New York City and Long Island is seeking Licensed Practical Nurses (LPNs) to provide in-home care for pediatric and adult patients. Compensation is $40 per hour with flexible scheduling options including full-time, part-time, and per diem.

Qualifications

• Active New York State LPN license.
• Minimum 1–2 years nursing experience (Home Care, Pediatrics, or Geriatrics) preferred.
• Current CPR/BLS certification.
• Experience with Electronic Health Records (EHR) preferred.

Responsibilities

• Provide skilled nursing care including medication administration, wound care, catheter care and IV therapy.
• Conduct patient assessments and contribute to individualized care planning.
• Monitor patient conditions and report changes promptly.
• Educate patients and family members regarding treatment plans and health management.
• Maintain accurate and timely clinical documentation in compliance with standards.
• Ensure patient safety while upholding nursing standards.
• Collaborate with interdisciplinary teams and community providers.

• Strong clinical judgment and ability to work independently
• Current CPR/BLS Certification
• Experience with Electronic Health Records (EHR) preferred
• Excellent communication, organizational, and interpersonal skills
• Reliable transportation and/or valid driver’s license preferred
