Job summary

Affirmed Home Care, Inc. is seeking a Pediatric Licensed Practical Nurse (LPN) to provide skilled nursing in the homes of medically fragile children across all five boroughs of New York City.

The role offers part-time or per diem hours, compensation of $40.00 per hour, with $80.00 per wound care case, and flexible scheduling.

Qualifications

• Current New York State LPN license – required.
• Minimum of one year of LPN experience – required.
• Pediatric nursing experience – strongly preferred.
• Fluent in English (verbal and written).
• BLS Certification – preferred.
• PALS Certification – preferred.
• Fully vaccinated against COVID-19 – preferred.
• Authorized to work in the United States (ITIN not accepted).

Responsibilities

• Provide individualized skilled nursing care to pediatric patients in their homes.
• Administer medications and treatments as prescribed.
• Monitor patient conditions and report significant changes promptly.
• Manage specialized medical equipment, including ventilators.
• Collaborate with physicians, caregivers, and interdisciplinary healthcare teams.
• Educate families on patient care procedures and safety measures.
• Maintain accurate and timely clinical documentation in accordance with agency standards and regulatory requirements
