WeCare Centers in New Castle, PA is seeking a Licensed Practical Nurse (LPN) to join our caring team. The LPN will monitor patient health, administer medications, assist with daily activities, and collaborate with the nursing team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.

• Provide regular patient status updates.

• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

• Complete required documentation in an accurate and timely manner.

• Supervise, administer and or assist with medications.

• Provide direct care of residents as necessary.

• Follow the plan of care for each resident.

• Ensure the residents care standards are maintained.

• Report to the Director of Nursing.
