Job summary

WeCare at Heritage Care Center in Pittsburgh, PA is seeking dedicated LPNs to join our growing team. This full-time night shift (11p-7am) position offers an opportunity to provide essential nursing care, monitor patient health, administer medications, and support daily activities within a collaborative healthcare team.

• Experience in long-term care or hospitals is a plus.

Responsibilities

• Conduct thorough patient assessments and monitor health status.
• Administer medications and assist with daily living activities.
• Collaborate with RNs and the care team to implement treatment plans.
• Complete accurate and timely nursing documentation.
• Supervise and assist with medication administration as directed.
• Provide direct patient care and support residents’ needs.
• Follow individualized care plans and maintain care standards.
• Report to the Director of Nursing and participate in team communication.

WeCare at Heritage Care Center, located at 5701 Phillips Avenue, Pittsburgh, PA, 15217 is actively seeking dedicated, LPNs to add to our growing team!

We are looking for fulltime 11p-7am

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.

Qualifications

• Valid LPN license in the State of PA
