Job summary

Agape Care Group is seeking a Licensed Practical Nurse (LPN) for our Lexington Hospice House. As an LPN, you’ll perform patient visits, deliver routine care under RN supervision, and collaborate with the care team and families to ensure compassionate, patient-centered experiences.

This PRN Day Shift role offers extensive leadership support, professional development opportunities, and a strong culture of integrity and care within a growing hospice network.

Qualifications

• Heart to serve patients and families.
• Graduate of accredited nursing program with current LPN license.
• 2+ years nursing experience in acute care or hospice setting preferred.
• Reliable transportation and ability to lift 80–100 lbs.

Responsibilities

• Conduct patient visits under RN supervision.
• Provide routine nursing care and coordinate with care team.

We are looking for hospice licensed practical nurses (LPNs) who are committed to creating meaningful patient experiences. As a hospice licensed practical nurse (LPN), you’ll work under the direction and supervision of the registered nurse to conduct patient visits and provide routine care, while communicating with the care team, the patient and their family.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a licensed practical nurse
• Experience: 2+ years of nursing experience in an acute care setting, or equivalent experience (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.
