Ascend Hospice in New Jersey is hiring a Licensed Practical Nurse (LPN) for the South West Team to provide comprehensive nursing care in a hospice setting.

The role involves patient observation, symptom management collaboration with the Hospice Nurse, and ongoing communication with families and the care team. Prior hospice or long-term care experience is preferred, with strong communication skills and teamwork essential.

Qualifications

• Hold a valid and current LPN license in the state of operation.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.
• Strong verbal and written communication skills.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient’s status and symptom management at appropriate levels.
• Delivers physical and psychological care as prescribed by the attending physician, under supervision.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and other meetings as assigned.
• Maintains sufficient home care supplies for patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assists with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides as assigned.

Education:

Associate's Degree in a related field

Professional Experience:

• Hold a Valid and current LPN license within the state of operations
• 1 year of clinical experience
