Job summary

ACG Hospice is seeking a Registered Nurse (RN) for the Lexington Community Hospice House on a part-time day shift (7am-7pm). You will evaluate patients, develop care plans, and coordinate with patients, families, and the care team to ensure compassionate, quality care.

You’ll join a supportive leadership team with opportunities for professional development and advancement. The organization emphasizes meaningful patient experiences, staff collaboration, and a mission to provide compassionate care

Qualifications

• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• Heart to serve patients and families with a passion for quality care.
• Reliable transportation to and from shifts.
• Experience: 2+ years of nursing in a clinical care setting; hospice experience preferred.

Responsibilities

• Evaluate patients and create care plans.
• Communicate with the patient, the patient’s family, and the care team.
• Serve as driver of the care team to ensure quality patient care.

Essential Functions

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
