As a Licensed Practical Nurse (LPN) at River’s Edge Rehab and Healthcare Center, you will provide hands-on nursing care to residents while supporting the clinical and leadership needs of the unit. You will collaborate with nursing staff and interdisciplinary teams to ensure residents receive compassionate, high-quality care.
Provide direct nursing care according to individualized care plans
Administer medications and treatments, ensuring accurate documentation
Perform clinical assessments and monitor changes in residents’ conditions
Assist physicians during rounds and exams
Communicate effectively with residents’ families and care team members
Ensure compliance with safety, health, and quality regulations
Foster a respectful, positive, and collaborative team environment
Current LPN license valid in PA
Graduate of an accredited LPN or vocational nursing program
Ability to work independently and collaboratively as part of a care team
Must pass TB screening and required health checks
Strong communication, organizational skills, and attention to detail
