Job summary

River Falls Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) to join our resident care team in a full-time, Monday–Friday day-shift role with a flexible schedule. The LPN will coordinate services, assess needs, and support residents in an assisted living community.

Ideal candidates hold a Massachusetts LPN license in good standing, with prior experience in related healthcare settings and a resident-centered approach.

Qualifications

• Currently licensed Massachusetts LPN in good standing.
• Experience in assisted living, long-term care, home health, or related healthcare settings preferred.
• Strong communication and organizational skills.
• Ability to work within an interdisciplinary care team.
• Compassionate, resident-centered approach.
• Ability to recognize changes in resident condition and report concerns promptly.

Responsibilities

• Assess resident needs and help develop and implement ISPs.
• Monitor residents' health status and communicate changes to providers.
• Coordinate care with residents, families, physicians, and service providers.
• Provide guidance to Resident Care Assistants and Supervisors.
• Assist with evaluating prospective residents during admission.
• Identify safety concerns and initiate corrective action.
• Serve as liaison among residents, families, staff, and providers.
• Promote quality, person-centered care while supporting independence and dignity.
• Perform other duties as assigned by the Wellness Director.

Full Time | Monday–Friday | Day Shift | Flexible Schedule

River Falls Senior Living is seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join our resident care team.

This is a full-time, Monday-Friday day-shift position with a flexible schedule. The LPN plays an important role in providing quality, person-centered care and coordinating services for residents in our assisted living community.

What You'll Do

• Assess resident needs and help develop and implement Individual Service Plans (ISPs).
• Monitor residents' health status and communicate changes to the appropriate healthcare providers.
• Coordinate care with residents, families, physicians, VNA/home health agencies, PT, OT, and other service providers.
• Provide guidance and support to Resident Care Assistants and Resident Care Supervisors.
• Assist with evaluating prospective residents as part of the admission process.
• Identify safety concerns and initiate appropriate corrective action.
• Serve as a liaison between residents, families, care staff, administration, and healthcare providers.
• Promote quality, person-centered care while supporting residents' independence and dignity.
• Perform other related duties as assigned by the Wellness Director.

What We're Looking For
