Job summary

Great Lakes Management seeks an LPN to join our Sun Prairie, WI team. This 24-hour-per-week, part-time role supports seniors in our assisted living community, requiring WI licensure.

The LPN will monitor care plans and coordinate with the team to maintain high-quality resident care. The successful candidate will administer medications, collaborate with the RN, and communicate changes to physicians and families, ensuring compliance with state regulations.

Qualifications

• WI LPN licensure required and current license in good standing.
• Minimum of one year in home health, assisted living or long-term care preferred.
• Good organizational skills with ability to work with minimal supervision.
• Caring demeanor with a passion for elderly care.

Responsibilities

• Ensure residents meet admission criteria and monitor care plans.
• Maintain up-to-date care plans per state regulations.
• Monitor resident charts and clinical progress notes.
• Communicate changes to physicians, staff and families.
• Coordinate resident services with other providers.
• Oversee medications and renewals, with physician follow-up as needed.
• Ensure proper medication administration procedures are followed.
• Collaborate with RN and care team; assist with emergency procedures.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
