Job summary

River Falls Senior Living is seeking a licensed practical nurse (LPN) to join our resident care team on a full-time, Monday–Friday day shift with a flexible schedule. The LPN will coordinate services, assess residents, and support a person-centered care approach in our assisted living community.

Ideal candidates hold a Massachusetts LPN license in good standing and have experience in assisted living or related settings.

Responsibilities

• Assess resident needs and develop ISPs.
• Monitor health and communicate changes to providers.
• Coordinate care with families, physicians, and services.
• Guide and support Resident Care Assistants and Supervisors.
• Evaluate prospective residents during admission.
