Job summary

Great Lakes Management is seeking an LPN to join our team in Sun Prairie, WI. This 24-hour-per-week, part-time role requires a WI LPN license and a passion for caring for seniors in assisted living.

Responsibilities include maintaining up-to-date care plans, monitoring residents, coordinating with physicians and staff, and ensuring accurate medication administration. We offer competitive pay ($30.00–$32.00 hourly), a 401(k) match, paid vacation and sick time, floating holidays, and a free meal

Qualifications

• Requires LPN license in Wisconsin.
• Minimum 1 year experience in home health, assisted living, or long-term care preferred.
• Good organizational skills with initiative and minimal supervision.
• Caring, patient-focused attitude toward elderly residents.

Responsibilities

• Maintain up-to-date care plans per state regulations.
• Monitor resident charts and progress notes.
• Coordinate with physicians and staff on changes in resident needs or conditions.
• Oversee medication administration and renewals; follow-up with physicians as needed.

LPN Job Details:

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident’s needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident’s log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate .
• Works with manager to assure emergency procedures are in place

LPN Qualifications :

• Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
