Job summary

Lifepoint Health's North Florida Rehabilitation Hospital in Jacksonville is seeking a Licensed Practical Nurse (LPN) for Rehabilitation. This full-time night position runs 6:45 PM–7:15 AM, with every-other-weekend coverage, delivering compassionate care in a dynamic rehab setting.

As an LPN, you will provide patient care, assist with assessments, administer treatments, document accurately, and support interdisciplinary rehab interventions.

Qualifications

• Graduation from an accredited LPN/LPN program.
• Current state LPN license.
• Minimum 1 year acute care LPN experience.
• CPR/BCLS certification; ACLS preferred.
• CRRN preferred.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Assists with patient assessments and contributes to care planning.
• Administers treatments and medications within scope of practice.
• Supports interdisciplinary care interventions focused on rehabilitation progress.
• Documents patient information accurately and in a timely manner.
• Provides care appropriate to the age and condition of the patient.
• Demonstrates professional judgment and patient advocacy.

How You'll Contribute

Provides patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse.
