Job summary

North Florida Rehabilitation Hospital is seeking a Licensed Practical Nurse (LPN) for full-time day shifts in Jacksonville, FL. The role emphasizes rehabilitation-focused patient care, with responsibilities spanning assessment support, medication administration, documentation, and interdisciplinary collaboration.

Qualified candidates have an accredited LPN program diploma, active Florida LPN licensure, CPR/BCLS certification, and ideally ACLS/CRRN credentials.

Qualifications

• Graduation from an accredited licensed practical nurse program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification is required; ACLS preferred.
• CRRN preferred and advantageous.

Responsibilities

• Provides patient care services that meet psychosocial, physical and general needs under RN supervision.
• Assists with patient assessments and contributes to care planning.
• Administers treatments and medications within scope of practice.
• Supports interdisciplinary care focused on rehabilitation progress.
• Documents patient information accurately and timely.
• Provides care appropriate to the age and condition of the patient.
• Demonstrates professional judgment and patient advocacy.

How you'll contribute

• Provides patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse.
• Assists with patient assessments and contributes to care planning
• Administers treatments and medications within scope of practice
• Supports interdisciplinary care interventions focused on rehabilitation progress
• Documents patient information accurately and in a timely manner
• Provides care appropriate to the age and condition of the patient
• Demonstrates strong clinical judgment, professionalism, and patient advocacy

Qualifications and requirements

• Graduation of an accredited licensed practical/ vocational nurse program.
• Current state licensure as Licensed Practical Nurse.
• A minimum of one-year acute care LPN Experience
• CPR/BCLS certification. ACLS preferred.
• CRRN Preferred
• Excellent oral and written communication and interpersonal skills.
