Job summary

Providence Rehabilitation and Healthcare Center at Mercy Fitzgerald is seeking a licensed practical nurse (LPN) to join a compassionate, patient-centered care team. Opportunities include full-time, part-time, and PRN roles with daily pay and starting wage of $32–$35 per hour.

Responsibilities include administering medications, following care plans, and collaborating with an interdisciplinary team to ensure high-quality resident care. Active PA LPN license and BLS CPR are required.

Qualifications

• Active LPN license in Pennsylvania (PA) is required.
• Valid BLS/CPR certification required.
• Ability to care for and manage multiple patients in a fast-paced healthcare setting.
• Strong clinical judgment and communication skills.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents' conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
