Job summary

Logan Square Rehabilitation and Healthcare Center is seeking a licensed practical nurse (LPN) for a part-time night shift (7 pm–7 am). The role includes comprehensive patient care, medication administration, and collaboration with an interdisciplinary team in a 109-bed Sub-Acute and Long-Term Care facility.

Candidates must hold a PA LPN license, current BLS certification, and be able to manage multiple residents in a fast-paced environment.

Qualifications

• LPN license active and in good standing in Pennsylvania.
• BLS/CPR certification required.
• Ability to care for multiple residents in a fast-paced setting.
• Good clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow individualized care plans with Nurse Manager and team.
• Administer medications and treatments as prescribed.
• Monitor residents, report changes promptly to supervisor and respond to needs.
• Maintain accurate documentation per orders, plans, and standards.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents' conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Pennsylvania.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
