Job summary

Graduate Post Acute in Philadelphia, PA seeks an LPN to join our compassionate team. Start pay ranges from $30 to $37 per hour, with daily pay options. We offer full-time, part-time, and PRN roles in a facility focused on resident well-being and high-quality care.

As an LPN, you will follow individualized care plans, administer medications, and collaborate with an interdisciplinary team to support residents’ physical and emotional needs. We value dignity, respect, and excellence in every shift.

Qualifications

  • Active LPN license in Pennsylvania, unencumbered.
  • Valid BLS (CPR) certification required.
  • Ability to manage multiple residents in a fast-paced setting.
  • Strong clinical judgment and communication skills.

  • Follow care plans developed by the Nurse Manager and interdisciplinary team.

Responsibilities for Licensed Practical Nurse (LPN):
  • Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
  • Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
  • Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
  • Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
  • Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
  • Support a safe, respectful, and therapeutic environment for residents and their families.
Qualifications for Licensed Practical Nurse (LPN):
  • Current, unencumbered active LPN license in the state of Pennsylvania.
  • Valid BLS Certification (CPR) required.
  • Ability to care for and manage multiple patients in a fast-paced healthcare environment.
  • Strong clinical judgment, communication skills, and attention to detail.
