Job summary

Springfield Rehabilitation & Healthcare Center is seeking Licensed Practical Nurses to join our 90-bed sub-acute and long-term care facility. We offer full-time, part-time, and PRN opportunities with daily pay and competitive hourly rates.

Earn $31/hr–$38/hr (all inclusive) and receive tuition reimbursement, referral bonuses, health/vision/dental, 401(k) with match, shift differentials, life insurance, and EAP resources.

Qualifications

• Current, unencumbered LPN license in the state of PA.
• Valid BLS Certification (CPR) required.
• Ability to care for multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow and implement individualized care plans developed by the Nurse Manager and team.
• Administer medications and treatments per the plan of care and facility policy.
• Monitor residents, report changes promptly to supervising staff, respond to needs.
• Maintain accurate, timely documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):
