Job summary

Belmont Bay Rehabilitation & Healthcare Center is hiring an LPN to join a compassionate team. Full-time and part-time opportunities are available, with shifts including 3 PM - 11 PM Fridays through Sundays and 11 PM - 7 AM every other Tuesday, Saturday, and Sunday, plus 7 PM start options.

Qualifications

• Active LPN license in Virginia (VA).
• Valid BLS (CPR) certification.
• Ability to manage multiple patients in a fast-paced setting.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow individualized care plans with Nurse Manager and team.
• Administer medications and treatments per policy.
• Monitor residents, report changes promptly to supervising staff.
• Document accurately and timely according to orders and standards.
• Maintain a safe, respectful, therapeutic environment.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of VA.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
