Job summary

The Brentwood Rehabilitation and Healthcare Center in Danvers, MA is seeking an LPN for full-time and PRN opportunities. Shifts include 7am–3pm for FT and 7am–3pm / 3pm–11pm / 11pm–7am for PRN with daily pay options and up to a $3.50 shift differential.

You will provide patient care per plan, administer medications, document accurately, and support a safe, respectful environment. A MA LPN license and BLS CPR are required.

Qualifications

• Current, active LPN license in Massachusetts.
• Valid BLS (CPR) certification required.
• Ability to care for multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per standards, plan of care, and physician orders.
• Follow care plans developed by Nurse Manager and team.
• Administer medications and prescribed treatments as planned.
• Monitor residents, report changes promptly to supervisor/physician.
• Maintain accurate documentation following orders and regulations.
• Support a safe, respectful environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Massachusetts.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
