Job summary

The Boulevard Post Acute in Virginia is seeking a licensed practical nurse (LPN) for PRN shifts. You will provide compassionate patient care, administer medications, follow care plans, and collaborate with an interdisciplinary team to support residents’ well-being.

Qualifications

• Current, unencumbered active LPN license in VA.
• Valid BLS certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow and implement individualized care plans developed by the Nurse Manager.
• Perform medication administration and prescribed treatments per the POC.
• Monitor residents’ conditions and report changes promptly.
• Maintain accurate, timely documentation per orders and standards.
• Support a safe, respectful environment for residents and families.

• Support a safe, respectful, and therapeutic environment for residents and their families.
