Qualifications

• Current Licensed Practical Nurse (LPN) license in Wisconsin.
• Graduation from an accredited nursing program.
• Long‑term care experience preferred.
• Knowledge of medication administration and nursing standards.

• Ability to supervise and coordinate Nursing Assistant assignments.
• Must pass background check per Wisconsin Caregiver Law.
• Valid Wisconsin driver’s license (if driving is required).

Under the supervision of a Registered Nurse, the LPN delivers direct nursing care, supports care planning, and helps ensure a safe, respectful, and person‑centered environment.

Key Responsibilities

• Provide nursing care within the Nurse Practice Act

• Administer medications and treatments under RN supervision

• Assist with training and orientation of new staff

• Observe, document, and report resident conditions and changes

• Support development and implementation of individualized care plans

• Assign and oversee work of Nursing Assistants

Physical Requirements

• Frequent standing, walking, talking, and hearing
