Lakeview Health Center & Assisted Living in Wisconsin is seeking a compassionate LPN to provide high-quality resident care in our long‑term care setting under RN supervision.

You will administer medications, support care planning, supervise Nursing Assistants, and contribute to person-centered care while maintaining resident rights and safety. This role offers competitive wages, shift differentials, and a supportive team environment.

Qualifications

• Current Licensed Practical Nurse (LPN) license in Wisconsin.
• Graduation from an accredited nursing program.
• Long‑term care experience preferred.
• Knowledge of medication administration and nursing standards.
• Strong communication and teamwork skills.
• Ability to supervise and coordinate Nursing Assistant assignments.
• Must pass background check per Wisconsin Caregiver Law.
• Valid Wisconsin driver’s license (if driving is required).

Responsibilities

• Provide nursing care within the Nurse Practice Act.
• Administer medications and treatments under RN supervision.
• Assist with training and orientation of new staff.
• Observe, document, and report resident conditions and changes.
• Support development and implementation of individualized care plans.
• Assign and oversee work of Nursing Assistants.
• Prepare shift reports and communicate updates to supervisors.
• Maintain resident rights, confidentiality, and safety.
• Participate in required in‑service education.
• Promote person‑centered care and positive resident interactions.
