Job summary

Aviata Health Group in West Palm Beach, FL is seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a family-oriented environment. You will provide nursing services, assist with treatment plans, supervise CNAs, and participate in Falls and Pressure Sore programs to ensure residents receive high-quality care.

Must hold an active LPN license; experience in Skilled Nursing/Rehabilitation facilities is preferred.

Qualifications

• Must possess a current, unencumbered LPN license.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Compassionate, dedicated and team-oriented approach.

Responsibilities

• Provide nursing services and contribute to individualized treatment plans.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore programs for residents.
• Conduct and document medical evaluations upon admission and during care.
• Perform other duties as assigned

Major Responsibilities

• Assisting in the implementation of individualized treatment plans for each assigned resident
• Supervising CNAs and participating in their evaluation
• Overseeing Falls Program and Pressure Sore Program
• Providing nursing services
• Conducting and documenting a thorough evaluation of each resident's medical status upon admission and throughout their course of treatment
• Other duties as assigned

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• Must be qualified, compassionate, and dedicated to a job well done
