Responsibilities

• Maintain accurate records of resident care, including medication administration and treatment plans.
• Communicate with residents, families, and healthcare providers to ensure coordinated care.
• Participate in quality care initiatives and contribute to individualized care plans.
• Collaborate with care team to provide comprehensive and coordinated care.

Education

• Participate in quality care initiatives and contribute to the development of individualized care plans
• Collaborate with other members of the care team to provide comprehensive and coordinated care

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of Missouri
• High school diploma or equivalent required; associate's or bachelor's degree in nursing or related field preferred
• Experience in long-term care or geriatric setting a plus
• Possess excellent communication, organizational, and problem-solving skills
