Job summary

Sans Souci Rehabilitation and Nursing Center is seeking an LPN to join our skilled nursing team in the Village of Tarrytown, NY. The role focuses on providing comprehensive resident care in a dynamic long-term care setting.

The LPN will administer medications (including IVs), monitor residents, and document outcomes while educating families and coordinating with physicians and NPs. Supervisory duties include overseeing CNAs and collaborating with a multidisciplinary team.

Qualifications

• Current LPN license is required.
• Experience in SNF/long-term care settings is helpful.
• Ability to monitor residents and respond to changes.

Responsibilities

• Administer medications, including IVs, and document accurately.
• Take and record residents' vital signs and report changes.
• Provide direct resident care and treatments with outcome monitoring.
• Educate residents and families about care plans and outcomes.
• Communicate with physicians, NPs, families, and health-care team; supervise CNAs.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Licensed Practical Nurse (LPN) QUALIFICATIONS/REQUIREMENTS:

• Current License-LPN

Salary: Up to $42 hour
