Job summary

Sans Souci Rehabilitation and Nursing Center is seeking an LPN to join our skilled nursing team. You will provide direct resident care on the Skilled Unit and Long Term Care Unit, administer medications, monitor vital signs, and educate residents and families on care plans.

The role includes collaboration with physicians and NPs, and supervision of CNAs. A current LPN license is required to practice in the state.

Qualifications

• LPN license is required to practice.
• Experience in SNF or long-term care is preferred.

Responsibilities

• Provide care for residents in Skilled and Long Term Care Units.
• Administer medications, including IVs where applicable.
• Take and record vital signs for residents.
• Provide resident care and monitor treatments and outcomes.
• Educate residents and families on care plan and procedures.
• Communicate with physicians, NPs, families, and other health professionals.
• Supervise CNAs to ensure quality care.
