Job summary

Sans Souci Rehabilitation and Nursing Center in Melrose, NY is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide hands-on resident care in the SNF and Long Term Care units, administer medications, monitor vitals, and collaborate with physicians, NPs, families, and other professionals.

This role includes supervising CNAs, educating residents and families, and contributing to positive outcomes through careful observation and documentation.

Qualifications

• Current LPN license.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Provide care for residents in the Skilled Unit and Long Term Care Unit.
• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
