Job summary

Water's Edge at Port Jefferson for Rehabilitation and Nursing is seeking an LPN to provide skilled nursing care in our SNF and long-term care units. The role includes medication administration and vital signs, with coordination of care and education for residents and families.

The position requires an active LPN license in New York, with opportunities to supervise CNAs and collaborate with a multidisciplinary team.

Qualifications

• Current NY LPN license is required.

Responsibilities

• Provide care for residents in the Skilled Unit and Long Term Care Unit.

• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
