Job summary

Grand Traverse Pavilions in Traverse City, MI is seeking a Licensed Practical Nurse (LPN) to join our care team. The base wage starts at $29.76 per hour with additional shift differentials for evenings and weekends and a wage pass-through option.

You will administer medications, monitor residents, support nurse aides, and help ensure high-quality, compassionate care while maintaining accurate records and complying with policies.

Qualifications

• Active LPN license in Michigan or eligible for licensure.
• Must pass criminal background check and drug screen.

Responsibilities

• Organize and supervise nursing care during assigned shift.
• Make resident rounds to monitor condition, safety, and equipment.
• Maintain care plans and inform supervisor of changes.
• Administer and document medications, manage controlled substances, and restock medications.
• Direct and perform treatments and procedures, document outcomes and charging procedures.
• Identify hazards to residents and initiate emergency measures as appropriate.

Position Details:

• Grand Traverse Pavilions is seeking a motivated, Licensed Practical Nurse (LPN)
• The base wage for this position is $29.76 / hour
• $5.00 / hour for pm shift and weekends
• $3.40 / hour Wage Pass Through
• Must be able to pass a criminal background check and drug screen.

Tasks:

• Responsible for organizing own work effectively, assisting the Nurse Aides to organize their work, and supervising the quality of nursing care on an assigned shift.
• Make resident rounds initially and regularly throughout the shift monitoring resident needs, change in condition, safety, proper functioning of equipment, employee performance, and follow through of physician orders.
• Assist in maintaining current care plans according to condition changes and advise supervisor accordingly.
• Administer and document medications, controlled drug accountability, ordering medications, and returning unused medications to pharmacy.
• Direct and/or perform and record procedures, treatments and response and complete charging procedures as required. Administer and document treatments per policy, and complete associated documents accordingly.
• Recognize and take appropriate action to eliminate or minimize a potential or actual hazard to a resident and to initiate emergency measures as appropriate.
• Report suspected infections to the Infection Control Coordinator.
• Check for proper placement of naso-gastric tubes and administer tube feeding as directed by physician order.
