Licensed Practical Nurse (LPN) Responsibilities

• Supervises Certified Nurse Aides under the direction of RN.
• Collects patient care data and monitors patient conditions and family dynamics based on observation, history, and nursing/medical diagnosis.
• Contributes to the development of the plan of care and discharge plan for the assigned patient.
• Implements patient care utilizing care plans, delegated medical orders, and collaborates with health team members as necessary.
• Performs administration and documentation of medications, nutrition, and treatments per physician’s orders and accurately records all care provided.
• Reviews medication and treatment records for completeness, accuracy in transcription of physician orders, and adherence to stop order policy.
• Utilizes the nursing process, documentation, communication, technical skills, and collaboration with physicians and other team members to meet performance requirements.

Licensed Practical Nurse (LPN) Qualifications

• Minimum requirements are graduation from an accredited school of licensed vocational nursing or licensed practical nursing with a current state license in good standing. Two or more years of acute care or rehabilitation experience as an LPN is preferred.
• Current state license to practice as an LPN. Must maintain current provider CPR certification throughout employment. ACLS certification is preferred.
