Position Details

Position: LPN Tele-Triage (Fully Remote)

Schedule: Part-time, Fridays, Saturdays & Sundays, 10am-6pm Central (11am-7pm Eastern)

Responsibilities

• Monitor phone lines and respond to caller needs promptly
• Assess symptoms and triage patients according to protocols
• Document all recommendations and notify operations
• Coordinate services with healthcare settings and discharge planning
• Manage medication orders and update relevant records
• Conduct assessments, develop care plans, and communicate with beneficiaries
• Ensure compliance with all regulatory requirements
• Provide health education and participate in performance improvement activities

Qualifications

• Degree/certification from an accredited nursing program
• At least one year of nursing experience
• Current LPN license in the state of practice with a compact license preferred
• Experience supporting individuals with disabilities or in home health/hospice care

Salary: USD $20.00/hour
