Location: Offsite - AdventHealth Home Health

Schedule: Full Time

Shift: Days & Nights, Every Other Weekend

Responsibilities

• Provide nursing services under RN supervision to patients in their homes

• Plan and coordinate patient care with RNs
• Assist with patient and caregiver education
• Respond flexibly to changing circumstances and prioritize accordingly

Minimum Qualifications

• 1 year of nursing experience as an LPN
• Experience with adult or geriatric populations
• Valid in-state Driver’s License with current insurance
• Active American Heart Association BLS certification
• Licensed Practical Nurse license in Florida

Preferred Qualifications

• 2+ years in Medicare-certified home health
• Home care experience
• Clinical background in medical/surgical or intensive care
