Job summary

Somerset Ridge (2) is seeking a compassionate Licensed Practical Nurse (LPN) to join our care team in Somerset, MA. You will provide direct resident care, follow care plans, document changes, and support a collaborative team to promote wellbeing.

Ideal candidates hold an active LPN license, demonstrate empathy and patience, and can lead CNAs and other care staff while adhering to facility protocols and safety standards.

Qualifications

• Must hold an active LPN license (state-specific).

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
