Job summary

Sarah S Brayton Healthcare Center in Fall River, MA is seeking a licensed practical nurse to provide compassionate resident care and collaborate with a dedicated care team.

You will conduct assessments, manage admissions and discharges, perform wound care, and lead CNAs and CMTs while following facility protocols. LPN licensure is required; strong integrity and empathy are essential.

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
