Reliant Care Management Company is seeking a dedicated LPN to provide compassionate bedside care for residents in Brockton, MA. You will assess, document, administer medications, and coordinate with physicians and guardians to ensure smooth admissions and discharges.

Qualifications

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
