Job summary

Autumn Grove Health Care Center Allens is seeking a dedicated LPN to join our care team. You will conduct assessments, perform admissions and discharges, and lead CNAs and CMTs while adhering to facility protocols. Your role includes wound care and emergency response as needed.

Responsibilities

• Conduct assessments, follow protocol, report changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, orders, and medications.
• Manage discharges: complete documentation and arrange transportation as needed.
• Provide wound care following protocols and dressings as required.
• Lead CNAs and CMTs while ensuring facility policies are followed.
• Respond effectively to emergency situations.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
