Job summary

Reliant Care Management, LLC is seeking an experienced LPN in Mansfield, OH to provide compassionate nursing support for residents. You will perform assessments, follow care protocols, document changes, and collaborate with the care team to ensure high-quality resident care.

The role includes admission duties, discharge planning, wound care, and supervising CNAs and CMTs while maintaining facility protocols.

Qualifications

• LPN license and regulatory eligibility.
• Ability to assess and document patient conditions.
• Willingness to lead nursing aides and med techs.

Responsibilities

• Conduct assessments, follow protocol, report changes, and document findings.
• Perform admission duties: assessments, MAR, TAR, POS, chart setup, physician orders, and medications.
• Manage discharges: documentation, notify physician/guardian, arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and dressings.
• Lead CNAs and CMTs to ensure compliance with facility protocols.
• Respond effectively to emergency situations.

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
